What does it mean when a server goes ‘down’? Why does it go down?

First of all, the word “server” actually means two things:

Typically, when we say a server is down, what we mean is that the program that is designed to respond to requests from the network is not doing so. There are a number of reasons that could happen, and each of them call for a different solution. Here are some example:
The power to the computer it runs on was cut. There are many reasons this could happen, including but not limited to people pulling the wrong plug while doing maintenance, cleaning, etc. In that case, you have to restore power.
The network connection stopped working.
Maybe some component failed. Hard drives and cooling fans, for instance, contain bearings, which wear out, and even components that are entirely solid-state sometimes just break. Whatever; the solution obviously is going to involve turning it back on, and if it doesn’t do that, sending the machine out for repair.
The computer is on, but the operating system is not running.
The computer is on and the operating system is running, but the particular server program is not running. Maybe it crashed, or somebody stopped it by accident. Somebody needs to start it again.
And now the tricky case: you have a machine with perfectly fine network and power connection that is running the OS and your server program, but somehow the server program is just not doing the right thing. because it was asked to do something unusual.
For instance, a database server that seems to be working fine for months can suddenly slow down to the point where it appears to be completely irresponsible just because it received a request (in the case of database servers, we call it a “query”) that takes a very long time to process. Sometimes, server bugs may cause a server to become irresponsible without actually crashing, so it looks like the server is still running but it is doing nothing useful. It is even possible that some hardware component is failing in a way that does not cause anything to turn off or crash, but instead just behave strangely. This is rare, but in a big enough installation you are going to see it.
For instance, a memory chip might be returning incorrect data, and if the memory has no or inadequate error-correction, the server program that cannot rely on correct data coming from memory might end up failing in just about any way.
